Current Research on Eating Disorders:

What may underlie differing perceptions of family functioning between mothers and their adolescent daughters with eating disorders? Daughters with eating disorders (ED) were found to perceive family functioning more negatively than their mothers. This study examined variables that may underlie these differences in a subgroup of adolescent girls and their mothers from the original study. METHODS: Participants were 77 mother-adolescent daughter dyads. Daughters met current DSM-IV criteria for Anorexia Nervosa, Bulimia Nervosa or ED Not Otherwise Specified. Daughters and mothers completed the Family Assessment Device (FAD), which measures perceptions of family functioning with seven subscales. Daughters also completed the Eating Disorders Inventory (EDI). Backward elimination stepwise regressions were conducted on difference scores between mothers and daughters for each FAD subscale. Daughter's EDI subscales (except Bulimia) were entered as possible predictors. RESULTS: Interpersonal Distrust was the only predictor of the differences between daughters and mothers on Problem Solving, Communication, Affective Responsiveness and General Functioning, accounting for 24%, 14%, 10% and 23% of the variance, respectively. CONCLUSIONS: Differences in viewpoints between mothers and daughters regarding the family environment may be understood in terms of the daughters' level of distrust of people in general and their overall feelings of inadequacy. Addressing these psychological difficulties may have powerful implications both for individual and family treatment in the recovery from an eating disorder. Could this be part of the dynamic that led to the eating disorder, a result of the stress on the whole family related to the eating disorder or some combination? Int J Adolesc Med Health. 2006 Apr-Jun;18(2):281-6.

Suicidality in eating disorders: Occurrence, correlates, and clinical implications. This review summarizes the published studies on suicide and suicide attempts in individuals with eating disorders, highlighting rates of occurrence, clinical correlates, and implications for practitioners. Multiple studies find high rates of suicide in patients with anorexia nervosa (AN) [Standardized Mortality Ratio (SMR) for suicide range from 1.0 to 5.3], whereas suicide rates do not appear to be elevated in bulimia nervosa (BN). In contrast, suicide attempts occur in approximately 3-20% of patients with anorexia nervosa and in 25-35% of patients with bulimia nervosa. Clinical correlates of suicidality in eating disorders include purging behaviors, depression, substance abuse, and a history of childhood physical and/or sexual abuse. Patients with eating disorders, particularly those with comorbid disorders, should be assessed routinely for suicidal ideation, regardless of the severity of eating disorder or depressive symptoms. Clin Psychol Rev. 2006 Jul 26.

Online Interactive Recovery Tools:

Childhood Trauma and Recovery from Eating Disorders. One of the long-term consequences of childhood trauma can be a lifelong struggle with compulsive and addictive behaviors.  It is not hard to understand why survivors of physical or sexual abuse would turn to food, alcohol, drugs, gambling or sex for comfort. Compulsive behaviors can keep trauma victims trapped in a vicious cycle, as their day to day lives are dominated by self-loathing and other painful emotions.  Over the course of a lifetime, survivors may suffer from a range of addictive or compulsive behavior, at any given time many are "cross-addicted". Trauma victims experience a much higher rate of eating disorders than the general population.  Unfortunately, some therapists fail to understand the implications of that statistical relationship, and end up telling their clients, "You have an eating disorder.  Therefore you must have been abused."  Such statements tend to invalidate the client's experience, undermine the therapeutic relationship, and lead to negative therapeutic outcomes (e.g., false "recovered memories"). Therapists who wish to explore childhood trauma need to stick closely to the data their clients provide, rather than offering their own scenarios or hypotheses.  Instead of spending a great deal of treatment time trying to put together a detailed historical narrative, it may be far more beneficial for long-term recovery to help the client work on the psychological issues that have resulted from the trauma, such as low self-esteem and pervasive self-blame. By addressing these issues, clients may be able to better control the triggers of binge eating, purging, and other compulsive behaviors.
