Current Research and News:
Update on course and outcome in eating disorders. Objective: To
review recent studies describing eating disorder course and outcome.
Method: Electronic and manual searches were conducted to identify
relevant articles published since 2004. Results: Twenty-six articles
were identified. For anorexia nervosa (AN), most patients ascertained
through outpatient settings achieved remission by 5-year follow-up.
Inpatient treatment predicted poor prognosis as inpatient samples
demonstrated lower remission rates. Outcome differed between bulimia
nervosa (BN) and eating disorders not otherwise specified (EDNOS),
including binge eating disorder (BED), for shorter follow-up durations;
however, outcomes appeared similar between BN and related EDNOS by
5-year follow-up. Greater psychiatric comorbidity emerged as a
significant predictor of poor prognosis in BN, whereas few prognostic
indicators were identified for BED or other EDNOS. Discussion: Results
support optimism for most patients with eating disorders. However, more
effective treatments are needed for adult AN inpatients and
approximately 30% of patients with BN and related EDNOS who remain ill

Are Diagnostic Criteria for Eating Disorders Markers of Medical
Severity? Objective: The objective of this study was to compare the
medical severity of adolescents who had eating disorders not otherwise
specified (EDNOS) with those who had anorexia nervosa (AN) and bulimia
nervosa (BN). Methods: Medical records of 1310 females aged 8 through 19
years and treated for AN, BN, or EDNOS were retrospectively reviewed.
Patients with EDNOS were subcategorized into partial AN (pAN) and
partial BN (pBN) when they met all Diagnostic and Statistical Manual of
Mental Disorders, Fourth Edition criteria but 1 for AN or BN,
respectively. Primary outcome variables were heart rate, systolic blood
pressure, temperature, and QTc interval on electrocardiogram. Additional
physiologically significant medical complications were also reviewed.
Results: A total of 25.2% of females had AN, 12.4% had BN, and 62.4% had
EDNOS. The medical severity of patients with EDNOS was intermediate to
that of patients with AN and BN in all primary outcomes. Patients with
pAN had significantly higher heart rates, systolic blood pressures, and
temperatures than those with AN; patients with pBN did not differ
significantly from those with BN in any primary outcome variable;
however, patients with pAN and pBN differed significantly from each
other in all outcome variables. Patients with pBN and BN had longer QTc
intervals and higher rates of additional medical complications reported
at presentation than other groups. Conclusions: EDNOS is a medically
heterogeneous category with serious physiologic sequelae in children and
adolescents. Broadening AN and BN criteria in pediatric patients to
include pAN and pBN may prove to be clinically useful. Pediatrics. 2010

Advocacy:
U.S. Sen. Harkin: Harkin, Klobuchar, Franken introduce bill to
confront eating disorders in the U.S. 4/27/2010 WASHINGTON,
D.C. – Senators Tom Harkin (D-IA), Amy Klobuchar (D-MN) and Al Franken
(D-MN) introduced legislation today aimed at fighting and preventing
eating disorders in the United States. The Federal Response to Eliminate
Eating Disorders (FREED) Act is the first comprehensive legislative
effort introduced in the Senate to confront the seriousness of these
diseases and to jump start research as well as improve the prevention,
screening, diagnosis and treatment of eating disorders. It expands
federal research, improves tracking and recording of the actual numbers
of people suffering and dying from these diseases, provides training for
a wide array of health professionals and educators to better identify
and screen for eating disorders, creates a new patient advocacy program
to help patients get proper care, authorizes grants for eating disorder
prevention programs and builds on the mental health parity and health
care reform bills to improve access to treatment, particularly for teens
covered by Medicaid.
To confront the growing issue of eating disorders, the FREED Act will:
* Expand research on the prevention of and effective treatment of eating
disorders: Coordinates research on eating disorders at the National
Institutes of Health and across the federal government, and creates
research consortia to examine the causes and consequences of eating
disorders, and to develop effective prevention and intervention
programs.
* Improve the training and education of health care providers and
educators: Authorizes grants to medical, nursing, social work and other
health professions schools to train health care providers in the
identification and treatment of eating disorders, and grants to train
teachers and other educators in effective eating disorder prevention,
detection and assistance strategies.
* Improve surveillance and data collection systems for tracking the
prevalence and severity of eating disorders: Tasks CDC with addressing
the lack of accurate information on the incidence and severity of eating
disorders. Requires the development of new methods to accurately
collect, analyze and report epidemiological data to ensure that the
incidence of eating disorders and related fatalities are better
understood.
* Prevent eating disorders: Authorizes grants to develop evidence-based
prevention programs and promote healthy eating behaviors and in schools,
recreational sports programs and athletic training programs.
* Build on existing reform efforts to ensure that treatment is available
and affordable: Creates a patient advocacy program to aid people
suffering from these diseases and their families negotiate the health
care system. Incentivizes states to ensure that adolescents covered by
Medicaid are diagnosed and treated.

Partial Hospitalization Program operates 11 hours per day, 7 days per

Dialectical Behavior Therapy (DBT) Center For Eating
Disorders. Dialectical Behavior Therapy is an empirically based
treatment for people who self injure. Recent evidence shows that DBT is
effective for eating disorders as well. As eating disorder specialists
for over 20 years, we know that ED’s can be treatment resistant often
necessitating multiple hospitalizations. This unique approach
incorporates traditional CBT with eastern mindfulness practices
providing a balance between acceptance and change. DBT’s bio-social
model frames eating disorders and other problem behaviors as resulting
from a biological predisposition to extreme emotional dysregulation in
combination with an invalidating environment where skills deficits
develop. Therefore, individual therapy is augmented by both weekly
skills group where new coping strategies are taught and phone coaching
which help clients generalize these new skills to real world challenges.
This multi-modal team approach provides clients with the enhanced
support they need across the entire week as they develop the ability for
emotional regulation, interpersonal communication and most importantly
distress tolerance. These are the very deficits that often interfere
with treatment and trigger self-destructive behavior and recurring

Oregon: Treatment Center News - RainRock Treatment Center. A
Residential Treatment Facility for Women, offers PHP, IOP, Outpatient,
and Transitional Living Programs. RainRock Treatment Center, A Monte
Nido Affiliate with Executive Director Carolyn Costin, LMFT, is pleased
to announce the opening of their Partial Hospitalization Program (PHP),
as well as their Intensive Outpatient Program (IOP). These new programs
are uniquely integrated into their existing 10 bed Residential Treatment
Center, providing the highest degree of continuity of care while
offering clients the experience of stepping down into the community.
This innovative program design allows their clients to stay connected to
their treatment team beyond their residential visit. This provides an
exceptional opportunity to maintain and build upon the progress achieved
during their residential stay. Living accommodations for both PHP and
IOP are offered at their new transition house, Turnberry Place. Located
next to RainRock, Turnberry Place is a warm and inviting 8 bed
transitional living house, where clients can practice day to day skills,

support of their treatment team. The hope in offering this continuum of
care is to give their clients the most comprehensive treatment available

Bellevue, WA: Treatment Center News - The Moore Center Opens New Residential Annex:
Located in the Seattle area, The Moore Center is Washington State's
longest-established eating disorders clinic, providing caring and
cost-effective treatment for adolescents, young adults and adults.
Services include: Partial Hospitalization Program (RIVA), an Intensive
Outpatient Program and several step-down levels of care. The Moore
Center is a fully licensed facility and is now able to treat patients
who do not live locally with the opening of the RIVA Residential Annex.
The Annex provides patients with a convenient lodging option while
participating in RIVA. Located less than a 3 minute walk from The Moore
Center, the two bedroom, two bathroom apartment with kitchen, dining,
and living rooms provides patients with a safe and convenient place for
patients to stay while receiving treatment. Pre-approved family members
