Current Research on Eating Disorders: Illness Intrusiveness in Anorexia Nervosa. Researchers: Carter JC, Bewell C, Devins GM. OBJECTIVE: "Illness intrusiveness" refers to illness-induced lifestyle disruptions. The primary aim of the current study was to compare the level of illness intrusiveness in anorexia nervosa (AN) to that reported in a variety of other chronic medical and psychiatric conditions. A secondary aim was to compare the two subtypes of AN (binge/purge vs. restricting) in terms of the nature and extent of illness intrusiveness. A final goal was to examine changes in the level of illness intrusiveness in AN following successful completion of specialized inpatient treatment. METHODS: The participants were a consecutive series of 121 female inpatients with AN who were admitted to a specialized inpatient unit for treatment of the eating disorder. Assessments took place before and after inpatient treatment and at 3-month follow-up. RESULTS: At baseline, illness intrusiveness scores for AN patients were significantly higher than those reported by women in the medical and psychiatric comparison groups. Overall, illness intrusiveness scores decreased (i.e., improved) significantly following successful completion of inpatient treatment. Among patients with the restricting subtype, scores continued to improve during follow-up, whereas this was not the case among patients with the binge-purge subtype of AN, whose scores did not change significantly during follow-up. CONCLUSION: Despite being notoriously ambivalent about change, these findings suggest that AN patients perceive their illness to be highly disruptive to a variety of life domains, even more so than patients with other chronic medical and psychiatric conditions. PMID:18440405. J Psychosom Res. 2008 May;64(5):519-26. The clinical significance of amenorrhea as a diagnostic criterion for anorexia nervosa. OBJECTIVE:: Amenorrhea is a DSM-IV criterion for the diagnosis of anorexia nervosa (AN). Several studies have reported few differences between patients who meet the full DSM-IV criteria for AN and those who meet all but the amenorrhea criterion. Although this suggests that the absence of menses does not provide critical diagnostic information, many of these studies are limited by small sample sizes. This study aims to examine the clinical utility of amenorrhea as a criterion for the diagnosis of AN. METHOD:: A chart review was conducted of 240 consecutive patients admitted for inpatient treatment at the NY State Psychiatric Institute from 1993 to 2006. Menstrual data were collected from the Eating Disorder Examination conducted upon admission. Independent samples t-tests were performed to evaluate differences in clinical variables, including age, lifetime lowest body mass index (BMI), admission and discharge BMI, previous number of hospitalizations, duration of illness, Beck Depression Inventory total score, Beck Anxiety Inventory total score, and Eating Disorder Examination subscale scores. RESULTS:: The amenorrheic and menstruating groups differed significantly only on lowest lifetime BMI and admission BMI, with individuals with amenorrhea having lower BMIs on both measures. CONCLUSION:: These results indicate that amenorrhea does not distinguish between groups on a number of important measures of clinical severity. It may be that amenorrhea reflects weight and nutritional status, rather than providing useful diagnostic information. Future studies are needed to examine the potential prognostic value of menstrual status. Int J Eat Disord May 2008. Nevada: Reno, NV: Treatment Center News - Center for Hope of the Sierras -- Alumni Intensives As a Residential Therapy program treating eating disorders, we realize that patients do experience lapses and relapses following residential treatment. The follow-up Alumni Intensives allow additional support to the patients following their departure from the Center. The follow-up intensives are individualized to the patients' needs, and are offered as an adjunct to outpatient treatment. Through these intensives, we aim to help patients understand that lapses are common following residential treatment, and to help the patient view lapses as further opportunity for growth, rather than as a sign that they have either failed treatment or that treatment has failed them. By helping the patient to see lapses as an opportunity for growth, the feelings of failure or shame are diminished. North Carolina: Durham, NC: Treatment Center News - Carolina House: In collaboration with Duke Medical Center's Eating Disorders Program, Carolina House is excited to offer Acceptance and Commitment Therapy (ACT). ACT is a new behavior therapy that has been shown to be effective for many issues, including eating disorders, and Carolina House is one of the few residential treatment centers to include this type of therapy. ACT psychotherapy looks at specific thoughts/feelings which may be keeping us from acting and being more flexible, free, and living how we'd prefer. ACT differs from more traditional cognitive-behavioral therapies in that it does not aim to eliminate "negative" thoughts or feelings. Instead, ACT emphasizes effective behaviors despite "negative experiences" in an effort to continue to live a valued and rich life. As is true of many of the contemporary "third wave therapies" ACT teaches mindfulness: how to be in the present moment instead of living in historical or fabricated realities that your mind creates. With the integration of ACT, Carolina House continues to strengthen its commitment to healing through mindfulness based psychotherapy. For additional information on the Carolina House treatment program and the services offered call 919-372-7951 or visit www.carolinaeatingdisorders.com. Business Building and Marketing Ideas:Make it easy: If you want more referrals, make it easy for people to contact you and then return calls quickly. Many times we call professionals only to discover that we are put on hold or we reach a terrible answering service or we find a large organization that has no idea about how to help us. Test out your phone system Monday morning. Make sure the person or machine answering your phone is clear, warm and calming. If you have a hard time leaving a message, then your clients will too. If you have an answering service, then do not call the special "call-in" number, call the number you give to clients. Make sure your first impression is a great one. It sounds so simple, but you would be surprised at what we find when we call most professionals. Read our other Marketing ideas by clicking here.
