Current Research and News:

Self-criticism is a key predictor of eating disorder dimensions among inpatient adolescent females. OBJECTIVE: Although the unipolar depression-eating disorder comorbidity is adequately documented, examination of the role of depressive personality styles in eating disorders is relatively scarce. METHOD: Associations between depressive symptoms, depressive risk and resilience (i.e., dependency, self-criticism, and sense of efficacy), and eating disorder symptoms (as measured by the Eating Disorder Inventory-2) were examined in inpatient adolescent females (N = 81). RESULTS: Self-criticism emerged as independent, robust, and strong predictor of eating disorder symptoms. CONCLUSION: Patients self-criticism should be targeted in psychotherapy and might serve as an obstacle for successful inpatient treatment. The role of self-derogation in eating disorders should be examined further. Source: Int J Eat Disord. 2008 Jun 20.

Time to remission for eating disorder patients: a 2(1/2)-year follow-up study of outcome and predictors. The aim of the present study was to analyze outcome, time to remission, and predictors of time to remission in a cohort of Danish eating disorder patients. Seventy-eight patients (35 anorexic, 30 bulimic and 13 unspecified eating disorder patients) were interviewed 2(1/2) years after initial assessment. Method of assessment was Eating Disorder Examination (EDE), Longitudinal Interval Follow-up Evaluation of Eating Disorders (LIFE-EAT-II), Eating Disorder Inventory (EDI), Symptom Check List (SCL-90R), Present State Examination (PSE) and the Structured Clinical Interview for DSM-III-R Axis-II (SCID-II). Method of analysis was Kaplan-Meier estimate of survival, Log Rank test and Cox regression analysis. In total 48.7% reached remission with mean time to remission at 27 months. A trend difference between the diagnostic groups when measuring time to remission was found, i.e. patients with unspecified eating disorders remitted faster than bulimic (BN) patients who in turn remitted faster than anorexic (AN) patients. Body mass index (BMI) at baseline was the best predictor of time to remission for the total sample. Predictors differed when looking at diagnostic groups separately. Final outcome was comparable with earlier studies while relapse frequency was low. Patients with AN remitted faster than found in earlier survival analysis studies, while the remission rate for BN patients was comparable with earlier studies. Despite the prognostic value of BMI for the total sample, predictor analysis implied more disorder diversity than homogeneity. Source: Nord J Psychiatry. 2008;62(2):151-9.

Judge Tentatively Approves Settlement of Eating-Disorder Class Action: A federal judge in New Jersey granted preliminary approval Tuesday to a class action settlement that would require Aetna Insurance Co. to improve coverage for some eating-disorder patients. U.S. District Judge Faith Hochberg made the decision after the plaintiffs' class action lawyer said the pact was good for the insured's and that litigating for a better deal would be risky. The settlement requires the company to treat some claims for anorexia and bulimia as it does claims for biologically based mental illnesses, such as schizophrenia. BBMI claimants are eligible for months of treatment, while those with non-BBMIs are limited by Aetna to 20 outpatient visits per calendar year and 30 days for inpatient benefits, according to the complaint in DeVito v. Aetna Inc. The better treatment would cover patients in "fully insured" plans, which are plans funded by employers. Enrollees in self-funded plans, such as employee welfare and state worker health benefit programs, are not covered by the suit and would not automatically benefit from the more liberal treatment. Class plaintiffs lawyer Bruce Nagel told Hochberg that about half of the company's enrollees are in "fully insured" plans. Aetna also agreed to establish a process in which eating-disorder patients for whom coverage is denied can appeal to an internal panel that includes an eating-disorder specialist selected by the plaintiffs attorneys.

They find it is crucial to offer a Relapse Prevention Group while in Residential, combined with follow-up Alumni Intensives, which allow additional support to the patients following their departure from the Center. The focus of Relapse Prevention group is to prepare patients to sustain the gains made in treatment after they leave the Center for Hope of the Sierras. Patients learn how to make effective use of treatment gains in ongoing preparation for discharge. This group helps patients clarify the foundations of their recovery and develop proactive shifts, leading patients to cope adaptively and constructively with sources of stress that are potential triggers for relapse. A variety of modalities are utilized to achieve the objective of this group, including ongoing, nonjudgmental discussion of struggles, role play, experiential activities, identification of potential roadblocks to recovery, and exploration of treatment outcome factors that have been reported in research literature. The follow-up intensives are individualized to the patients' needs, and are offered as an adjunct to outpatient treatment. Business Building and Marketing Ideas:

Understand the value of each referral.  One referral can open the door to referrals from friends and family and can be "the tip of the iceberg," in terms of income. Each individual can lead to many thousands of dollars in income over the life of your practice. You should understand what each referral can mean to your bottom line which will in turn help you make decisions about the amount you want to spend for your advertising.

Understand the value of each referral source. Be sure to make an extra effort to thank the people who refer to you with a follow-up call, email, or a card. Something that says, "there is no compliment better than a referral, and I appreciate the trust you have placed in me."  Many times the person doing the referring is left wondering if anything happened. Don't let this take place in your practice. Small gestures can make a big difference in other's view of your treatment. Also sending some flowers during the holidays to your good referral sources can go a long way toward making you stand out in their minds when they are deciding to whom to refer.  Even if the referral source is giving out three names, they are always emphasizing one name over the others.  Make sure it is you.

Follow Success.  If you see a business repeatedly doing the same advertising, take notice. Nobody likes wasting money and if whatever your competition is doing is working over time, then consider that this advertising may also be a good place for you to advertise.

Improve your web site placement with links: If you link to popular sites such as www.edreferral.com and we have links to your web page on our site, then it will help both of us to place higher in the Internet search engines. Don't underestimate the value of placing a link on your site to extremely popular eating disorder sites. One of the criteria the search engines use is the quality of relevant links on your site.
