Living with the Anorexic Voice: A Thematic Analysis.  Objective: A factor said to drive the behaviours of people with the eating disorder anorexia nervosa is the inner voice some recount hearing. However, little systematic examination has been made of this entity. The study aimed to investigate experiences of and reflections on living with an anorexic voice. Design: A qualitative approach was used because the study sought to establish the perspectives of those with anorexia who identified with the concept of living with an anorexic voice. Method: Individuals from three self-help organizations were invited to write about their life with an anorexic voice in the form of a poem, a reflection, a letter, or a descriptive narrative. Recruitment continued until data saturation was reached. Thematic analysis was employed by two researchers. Results: Written contributions were provided by 21 participants. These data underlined the positive and negative attributes individuals bestowed upon their anorexic voice; the former appeared stronger during the early stages of their eating disorder, the latter coming into force as it developed. In spite of their voice's harsh and forceful character, participants felt an affiliation towards it. Discussion: The bond between individuals and their anorexic voice could explain their ambivalence to change. Therapists must persist in their endeavours to penetrate this tie, whilst acknowledging the hold this entity has over those with anorexia. Diagnostic Crossover and Outcome Predictors in Eating Disorders According to DSM-IV and DSM-V Proposed Criteria: A 6-Year Follow-Up Study. Objective: To evaluate in a 6-year follow-up study the course of a large clinical sample of patients with eating disorders (EDs) who were treated with individual cognitive behavior therapy. The diagnostic crossover, recovery, and relapses were assessed, applying both Diagnostic and Statistical Manual of Mental Disorders, Fourth Edition (DSM-IV) and the DSM-V proposed criteria. Patients with EDs move in and out of illness states over time, display frequent relapses, show a relevant lifetime psychiatric comorbidity, and migrate between different diagnoses. Method: A total of 793 patients (including anorexia nervosa, bulimia nervosa, binge eating disorder, and EDs not otherwise specified) were evaluated on the first day of admission, at the end of treatment, 3 years after the end of treatment, and 3 years after the first follow-up. Clinical data were collected through a face-to-face interview; diagnosis was performed by means of the Structured Clinical Interview for DSM-IV and the Eating Disorder Examination Questionnaire was applied. Results: A consistent rate of relapse and crossover between the different diagnoses over time was observed. Mood disorders comorbidity has been found to be an important determinant of diagnostic instability, whereas the severity of shape concern represented a relevant outcome modifier. Using the DSM-V proposed criteria, most patients of EDs not otherwise specified were reclassified, so that the large majority of ED patients seeking treatment would be included in full-blown diagnoses. Conclusions: Among EDs, there are different subgroups of patients displaying various courses and outcomes. The diagnostic instability involves the large majority of patients. Eating Recovery Center is pleased to announce the opening of their new specialty hospital for children and adolescents with eating disorders in late November 2010. They are currently accepting patients for admission to their facility. The child and adolescent hospital will treat children and adolescents ages 10-17, including boys and young men. Ovidio Bermudez, MD, FAAP, FSAM, FAED, CEDS, will serve as the hospital’s Medical Director. It will operate under the direction of Eating Recovery Center’s CEO and Co-Founder, Kenneth Weiner, MD, FAED, CEDS and Chief Clinical Officer, Craig Johnson, PhD, FAED, CEDS. “Our comprehensive treatment model will blend traditional approaches like medical stabilization, psychiatric stabilization and nutritional rehabilitation with new approaches like Behavioral Family Therapy in the partial hospitalization phase of the treatment experience,” explains Dr. Bermudez. “We have carefully chosen an outstanding staff and, in addition, will use technologies to enhance patient care. Reno, NV: Treatment Center News -  Center for Hope of the Sierras – New IOP & PHP Day Treatment Programs Opening January 2011. Center for Hope of the Sierras is proud to announce planned program expansion! As of January 2011 they will be offering multiple levels of care to better meet the needs of their local and regional communities. In addition to the proven residential and unique 24-hour partial hospitalization programs, Center for Hope will soon be offering partial hospitalization day treatment (PHP) and intensive outpatient therapy (IOP) to women and adolescents, for the treatment of anorexia nervosa, bulimia nervosa, binge eating and related disorders including a specialized track for the treatment of co-occurring diabetes and eating disorders. The new IOP will require patients to participate in programs four hours per day, three to five days a week, with both afternoon and evening sessions available. The PHP day treatment will consist of eight hours per day, five days per week. Both levels of care incorporate many of the same components as residential treatment at the Center for Hope including a variety of individual and group therapy options, nutritional counseling with a registered dietitian, individualized treatment planning, medical management, and meal support. Manassas, VA: Mary Mannhardt Ministries will be implementing a 12 week multi-disciplinary Intensive Outpatient Program for women and teens suffering with anorexia, bulimia, and binge-eating disorder. It is geared for individuals who are ready for a step down structured program after hospitalization or for those looking for more than weekly therapy. Treatment modalities will include a wide variety of group therapies, both with a cognitive behavioral and Biblical perspective. Participants will utilize their creativity to heal through music, art, and experiential writing. Clients will be actively involved in groups for anxiety, body image, codependency, meal processing, nutrition, positive thinking, and spirituality. Inner strength and beauty, media imagery, and the development of healthy and constructive coping skills will all be discussed and explored. The goal of treatment is to gain a deeper understanding for how recovery from an eating disorder can contribute to a richer life experience and be part of the exciting discovery of a more purposeful life. How to Reduce No-Shows. When a caller is shopping around, he or she may not understand the appointment process. The caller may consider this appointment as similar to a dinner reservation (where many people feel that the business is probably so busy that one person not showing up will be hardly missed). The therapist needs to explain to the caller that this situation is unique and that your time will be “set aside for you (the caller).” You should explain to the caller that this appointment must be canceled 24 hours before the appointment or else the caller will be charged. In addition, the therapist should obtain some identifying information. While making an appointment with the caller, ask directly for his or her full name, address, and phone number. Then remind the caller about the charge for missed appointments. If the first appointment is more than 3 days ahead, mail the initial forms, directions to your office, and welcoming information. Then be sure to call the new client to give a reminder the night before the scheduled time. If the appointment is less than 3 days ahead, ask if a fax is available so that you can send the same information. These tips will help you to reduce the number of no-show clients.
