Current Research on Eating Disorders:

Treatment of anorexia nervosa: Insights and obstacles. Guarda AS. Department of Psychiatry and Behavioral Sciences, Johns Hopkins School of Medicine, Baltimore, MD. Anorexia nervosa is a behavioral disorder characterized by ego-syntonic self-starvation, denial of illness and ambivalence towards treatment. Treatment refusal and drop-out rates are high and relapse is common. Treatment is best viewed as comprised of two phases, weight restoration and normalization of eating behavior followed by relapse prevention. Most patients verbalize a desire to change, however they seek treatment on their own terms, ideally with minimal or no weight gain. Successful treatment must therefore convince patients to overcome their drive to diet. Evidence-based data on treatment interventions for anorexia nervosa are scarce and methodological problems afflict the few published, controlled trials. Taken together, clinical expertise and data from correlational and controlled trials suggest that chronicity and adult status are associated with a worse prognosis. Outpatient family therapy is effective in weight-restoring the majority of adolescent patients whereas older patients, or those with severe medical or psychiatric comorbidity, often require intensive treatment on an inpatient eating disorders behavioral specialty unit. Correlational data suggest that weight-restored patients are less likely to relapse. Despite limitations of the current knowledge-base, several new areas of research hold promise in elucidating risk factors, in identifying the pathophysiology that sustains anorectic behavior, and in developing more targeted and effective treatments. PMID: 18155737; Physiol Behav. 2007 Nov 22.

Treatment goal weight in adolescents with anorexia nervosa: Use of BMI percentiles. Researchers: Golden NH, Jacobson MS, Sterling WM, Hertz S. Division of Adolescent Medicine, Schneider Children's Hospital, Albert Einstein College of Medicine, New Hyde Park, New York. OBJECTIVE: There is a lack of consensus as to how to determine treatment goal weight in the growing adolescent with anorexia nervosa (AN). Resumption of menses (ROM) is an indicator of biological health and weight at ROM can be used as a treatment goal weight. This study determined the BMI percentile for age at which ROM occurs. METHOD: A secondary analysis of a prospective cohort study examining 56 adolescent females with AN, aged 12-19 years, followed every 3 months until ROM. BMI percentiles for age and gender at ROM were determined using the nutrition module of Epi Info 2002. RESULTS: At 1-year follow-up, 36 participants (64.3%) resumed menses and 20 (35.7%) remained amenorrheic. Mean BMI percentile at ROM was 27.1 (95% CI = 20.0-34.2). Fifty percent of participants who resumed menses, did so at a BMI percentile between the 14th and 39th percentile. CONCLUSION:: A BMI percentile range of 14th-39th percentile can be used to assign a treatment goal weight, with adjustments for prior weight, stage of pubertal development, and anticipated growth. Research Studies on Eating Disorders:

The National Institute of Mental Health (NIMH) is set to conduct a new long-term study of anorexia nervosa in 240 teens, ages 12 to 18. Researchers plan to start recruiting subjects on May 1 and will follow subjects for a four-year period. Six research facilities in the United States and Canada will work together with the NIMH on the new study, which will focus on early-intervention treatments. Experts believe that a combination of factors leads to anorexia. American society's worship of ultra-thinness places immense pressure on women - and on men.  Research has found that a woman with an anorexic mother or sister is more likely to become anorexic herself. Also, parents who are highly focused on appearance, frequently go on diets, or make negative comments about their children's bodies, are more likely to have a child who develops anorexia. Other factors which may lead to anorexia include trauma, a perfectionistic personality, and a genetic predisposition to the disorder. Getting help early on is crucial to effective treatment.

The study involves one visit of approximately 3 hours.  Their treatment study is for women aged 18-45 with anorexia nervosa and absent or irregular menstrual periods. They are investigating the combined use of a natural hormone and a medication that is effective for bone loss in postmenopausal women as a novel treatment for the bone loss seen in women with anorexia nervosa. They hope that the combination of these two investigational medications will help rebuild depleted bone and prevent further bone breakdown in women with anorexia nervosa.
