Insurer Agrees to Expanded Coverage for Eating Disorders: Insurer to Classify Eating Disorders as Biologically Based Illnesses. Nov. 26, 2008: The largest health insurance provider in New Jersey has agreed to settle a class action lawsuit brought by families who claimed they were denied adequate coverage for eating disorders. Under the tentative agreement, which still must be approved by the federal judge overseeing the case, Horizon Blue Cross Blue Shield of New Jersey will now treat eating disorders as biologically based illnesses, which will entitle patients to greater coverage. The settlement covers only the beneficiaries of fully insured plans. The lack of full coverage has been one of the main complaints of families of people who suffer from eating disorders, many of whom have had to pay tens of thousands of dollars out of pocket for treatment. Advocates and doctors said they hope the settlement, one of the first of its kind, will lead other insurance companies to take similar steps. In many states, insurance companies cover 30 or 60 days of hospital care a year. Only 13 states mandate coverage for at least some eating disorders, said Diane Robertson at the ECRI Institute, an independent nonprofit organization that tracks such legislation. Those policies, advocates say, have left families with massive debt. The relationship of bulimia and anorexia nervosa with bipolar disorder and its temperamental foundations. Researchers: Lunde AV, Fasmer OB, Akiskal KK, Akiskal HS, Oedegaard KJ. Department of Clinical Psychiatry Risskov, Aarhus University Hospital, Denmark; International Mood Center, University of California, La Jolla, CA, USA. BACKGROUND: Earlier studies have suggested a relationship between bipolar disorder (BP) and eating disorders (ED), more specifically, bulimia nervosa (BN) and bipolar II disorder (BP-II). In the present report we extend this relationship to broader definitions of bipolarity. METHODS: Semi-structured interview of 201 patients with DSM-IV criteria for major affective disorders combined with Akiskal and Mallya criteria for Affective temperaments. To diagnose lifetime comorbid eating disorders DSM-IV criteria for eating disorders (Bulimia Nervosa, BN, Anorexia, AN) were used. RESULTS: 33 patients had an eating disorder. When compared to patients without ED the patients with ED had a higher prevalence of bipolar disorders. Using strict DSM-IV criteria, this association was only significant for BN (OR) 4.5 (95% CI 1.1-17.6). When using a broader index of bipolarity including patients having affective temperaments, a significant relation was found for BN (OR) 9.1 (95% CI 1.1-73.6), and for patients with a lifetime history of both BN and AN (OR) 8.6 (95% CI 1.1-70.2).We also found patients with ED to have a significantly higher prevalence of affective temperaments, an earlier onset of major affective disorder and to have more depressive episodes. LIMITATIONS: Non-blind evaluation of diagnosis for mood, eating disorders and affective temperaments. CONCLUSION: In line with previous reports we describe an association between bulimia nervosa and bipolar disorder. Furthermore we report a relationship between lifetime bulimia and anorexia and cyclothymic and related affective temperaments. Source: J Affect Disord. 2008 Nov 11. Preventing eating disorders. This article reviews eating disorder (ED) prevention programs, highlighting features that define successful programs and particularly promising interventions, and how they might be further refined. The field of ED prevention has advanced considerably both theoretically and methodologically compared with the earlier ED prevention programs, which were largely psychoeducational and met with limited success. Recent meta-analytic findings show that more than half (51%) of ED prevention interventions reduced ED risk factors and more than a quarter (29%) reduced current or future eating pathology (EP). A couple of brief programs have been shown to reduce the risk for future onset of EP and obesity. Selected interactive, multisession programs offered to participants older than 15 years, delivered by professional interventionists and including body acceptance or dissonance-induction content, produced larger effects. Understanding and applying these results can help inform the design of more effective prevention programs in the future. Child Adolesc Psychiatr Clin N Am. 2009 Jan;18(1):199-207. Support Groups and Online Help: Online: December 16, 2008 Webinar – “What Every Parent Should Know About Outpatient Alternative Treatment for Eating Disorders” Featuring Dr. Renae Norton. “Save Her At Home.” Dr. Norton discusses outpatient alternative treatment for eating disorders. Concerned that your loved one has an eating disorder? Unsure about how to help her? Confused about inpatient versus outpatient treatment for eating disorders? Here is your chance to ask Dr. Norton about outpatient alternative treatments for eating disorders. Go to http://www.askdrnorton.com to ask your question about outpatient alternative treatment for eating disorders and to learn how you can attend the webinar. After you have submitted your question you will receive free access to Dr. Norton’s audio interview on “The Three Things Every Parent Should Know About Eating Disorders.” Dr. Norton is known as a “go to” therapist for some of the most severe cases of eating disorders in the Cincinnati area. “We approach the problem by aligning with the patient rather than colluding against her, blaming her, or casting her in the role of someone who is incapacitated. In the case of Anorexia, many practitioners frame the problem as a thought disorder, which justifies taking control away from the patient and forcing her into various treatment modalities. While those suffering from an eating disorder do have severe and/or debilitating distortions regarding food, weight and body image, they will ultimately have to choose for themselves whether or not to face their fears and change their relationship with food.” Go to http://www.askdrnorton.com to ask your question today. It is so important that your potential client's first contact with your office is a positive experience. I am always surprised at what a poor job professionals do of making this first contact as pleasant as possible. The worst way you could have your phone answered is with an answering service. Just recently I was actually put on hold for over 2 minutes waiting for an operator to answer. These services give the professionals a special number to call for messages so they never experience what poor service they are actually receiving. The next worse way to answer your phone is to have a busy staff member who has no clinical experience answering the phone. If this person is rushed and cold, the caller will not have a positive experience. I recommend having a high-quality answering system that projects your voice in a crystal clear manner. Then you can be calm and quiet and record a peaceful sounding outgoing message that is going to put the stressed-out caller at ease. Say on your outgoing message that the call is important to you and then make sure you call back promptly. Research your referral sources. Make a point of spending a few minutes with each new client finding out exactly how they found out about you. Try to dig a bit deeper than the initial response. If they say they found you on the Internet, then ask a bit more. What search engine did you use? Did you find my name on a particular website? Unfortunately, most therapists do not ask these questions. This research time is important to your business. This is vital information that will help you make future advertising decisions.
